We are dedicated to maintaining the privacy and security of our patient’s health records while maintaining the highest standards of care regarding how we handle and distribute patient information.
Our Medical Records Department, also known as Health Information Management, keeps a record of all the healthcare services we’ve given you.
Your medical records are kept in strict confidence, as required by state and federal law. The contents of your medical history will not be disclosed without your express consent or as required by law.
Call the Health Information Management Department at 360-814-8462 between 8 am and 4 pm Monday through Friday to get a copy of your medical record or send a written request to 1415 East Kincaid Street, Mount Vernon, WA 98273. Attention: Health Data Administrators
Access your medical records online
Skagit Regionals Health has a patient portal called MyChart, where you may join up to access your medical data.
Through MyChart, you get safe and easy access to your entire medical record, including lab and test results, current vaccinations, preventative care, and medical/surgical history. MyChart is always on and always accessible.
You have the right to a copy of your medical records.
A Patient Request for Health Information form is available for download for patients needing access to their medical records.
Complete this form in its entirety; thank you! Unprocessed forms that are missing information will be sent back to the requester.
The Authorization for Release of Health Information form may be obtained by a third-party requestor (such as a family member or personal representative) seeking access to protected health information.
Only the patient, the patient’s legal guardian, the parent of a minor patient, or the personal representative of a dead patient may approve the dissemination of healthcare information.
All forms must be completed in total, and unprocessed forms will be returned if they are incomplete.
Skagit Regionals Health will send your doctor free copies of your medical records. There will be a charge for any other kind of request, and rates may be discussed upon request.
