Our History, Our People, and Our Community
Lynn, Massachusetts, Essex County’s central city, is well-known for its industrial past and labor movement contributions.
Throughout the last two centuries, Lynn has been a welcoming place for immigrants from all over the world and people escaping slavery in the United States.
There is a wide range of people working at Lynn Community Health Center, all of whom share a commitment to social justice and decreasing health inequities in the community.
Lynn Community Health Center has always been committed to finding and filling up the gaps in healthcare for Lynn people.
Upon being recruited by the City of Lynn in 1971 to establish a health clinic, William Mantzoukas immediately hired Stephen D. Hayes, a freshly graduated psychologist with huge dreams and an eagerness to help people in need.
To address the paucity of mental health resources, they opened a small shop on Chestnut Street and named it the Community Counseling Center.
Having served 500 patients in our first year, it became clear that many of them could not get necessary primary care because of the many obstacles they faced.
After moving to a new location in 1976, we started offering medical and maternity services.
The staff of 650 and 150 clinicians
Daily, we have over 650 employees and more than 150 clinicians who treat more than 40,000 patients at our 25 sites throughout the city of Lynn, which includes our School-Based Health Centers.
In 1993, we relocated to our present facility at 269 Union Street, our primary location. From 62,000 to 283,000 patient visits annually since then. In recent years, LCHC has seen an average of 400 new patients a month.
We constructed the Stephen D. Hayes Building in 2012 to accommodate the increased demand. We now serve more than 40% of Lynn residents as a complete primary care provider.
In 1971, the City of Lynn engaged William Mantzoukas to start a health clinic there. A newly-minted psychologist with lofty dreams and an eagerness to help people in need, Stephen D. Hayes, was the first person he hired.
Together, they set out on the path that has led us to this point.
We serviced 500 patients in a small shop on Chestnut Street during our first year of operation. As soon as we began working with our patients, it became apparent that they were in dire need of primary care.
After moving to a new location in 1976, we started offering medical and maternity services. We’ve grown steadily to serve our community’s healthcare requirements over the years, and we now provide comprehensive primary care services to more than 40% of Lynn residents as the area’s most prominent provider.
Dr. Hayes started to see mental health treatments as an essential aspect of general care during these early years.
As a result of his groundbreaking work, the current Patient-Centered Medical Home Initiative, which is being extensively embraced at the state and federal levels, is based on his principles.
A pioneer in advocating the notion of treating the body, mind, and spirit as a whole was Dr. Hayes.
Bill Mantzoukas worked as the health center’s Executive Director for three years, and he has been a longtime friend and board member of the organization ever since.
We wouldn’t be where we are now without his foresight and guidance.
Dr. Hayes’ dedication to our patient’s physical and emotional well-being was unwavering. Throughout his 40-year career, he served as the Director of Behavioral Health and the Director of Integrated Care Services. Our new facility is named after him because of his incredible passion and commitment.

How to Get Access to Your Medical Records
If you’re a patient at Lynn Community Health Center, you have the right to see and acquire a copy of your medical records or to have copies of your medical records transmitted to another healthcare institution.
Before disclosing any documents, the health facility needs a signed and completed Authorization for the Release of Information Form.
There are several situations in which a patient’s doctor may additionally be required to provide their approval.
Individuals granted access to their medical records must pick them up in person. Documents released to another facility will be sent that facility’s way by postal mail.
How to Get a Copies of Your Medical Records
Please fill out the Authorization for Release of Health Information form and return it to the appropriate person.
The patient or their legal representative must sign and date the paperwork.

How can I set up a time to meet?
To better serve the people of Lynn, we have many locations where we provide a diverse range of services.
To schedule an appointment, contact our main number at 781-581-3900 or visit our hours/locations page to locate the phone number for a particular location.
When arranging a meeting, remember to:
- Be prepared with your insurance details.
- Know the purpose for the appointment (e.g., a physical, children’s vaccines, sick visit, Etc.) before you arrive
- Current patients should let our staff know that they are presently receiving care at the Health Center and give the name of their present doctor or another healthcare practitioner. They’ll put you on the calendar. If you can’t remember the name of your service provider, they’ll be happy to help you.
A provider appointment will be scheduled for you if you are a new patient at Lynn Community Health Center. Please have your insurance information available if you have it.
Your absence of insurance should be communicated to our personnel. Your Enrollment Coordinator will set up an appointment to meet with one of our Enrollment Specialists to see whether you are qualified for any of our public health insurance plans.
How can I get help now?
For minor illnesses and injuries that need medical care but are not life-threatening, the Lynn Community Health Center Urgent Care Center offers walk-in treatment at no cost.
If you have a significant or life-threatening problem, dial 911 or go to the closest hospital emergency room.
What do I need to do to prepare when I go to the doctor?
- Arrive early for your first appointment since you’ll need to fill out some paperwork. Over 15 minutes of lateness may necessitate rescheduling your work or maybe making you wait in the waiting room.
- Medications: Do not leave home without all of your prescriptions. Please bring the actual bottles or a list of the drugs, including their dose and frequency of use, to the appointment. OTC drugs are included in this.
- The following is a list of concerns: It’s a good idea to note any concerns or questions you have ahead of time and bring them to your appointment.
- Family/Friend: If you can, bring a friend or family member along for a visit.
What can we expect to see throughout the trip?
- The receptionist will ask you to sign in and complete some papers. Anyone who can help will do so.
- Assuming you have coverage, this information will be requested. So, please, don’t forget to bring it along.
- Our Enrollment Coordinator can help you if you do not have medical insurance.
- Our Nurses or Medical Assistants will next ask you a few questions about your medical history and measure your blood pressure and temperature in the exam room.
- A Doctor or Provider will then see you.
- If you have any questions or concerns regarding your health, medicine, or anything else, please don’t hesitate to ask. It’s essential to be aware of and educated about health-related problems.
- You will check out after your visit. These appointments will serve as the basis for any future ones.
Are you able to supply an interpreter?
On-site interpretation services are available in more than 15 languages, including Spanish and Russian.
You may utilize a telephone interpreting service if we don’t have an interpreter on-site who can speak your language. A medical interpreter is required if you want to meet with a doctor.
What should I do if I’m ill and the health facility is closed?
After hours, our doctors are on call and ready for consultation. When the health center is closed, patients may contact 781-581-3900, the facility’s primary phone number.
Our answering service will take these calls and forward them to the provider on call, who will get back to you as soon as possible.
