There is approximately 5000 personnel at Kaweah Health, including more than 1,000 licensed nurses, who serve the residents of California’s Central Valley with high-quality health care within a short drive away.

By using the URL https://www.hxe.io/hipaa form/kdhcd (What is hXe.io?) or by filling out our form entitled “Authorization for Use or Disclosure of Health Information” and submitting valid identification, patients may get electronic copies of their medical data.

How to access your health information

The new patient portal, “MyHealth Kaweah Health,” allows you to view your health records. You may now obtain your test results as soon as they are available using our new site.

All of our new patient portal invites are sent by email. There is a phone number and email address at the bottom of this page if you want to receive an invitation before your next visit.

Create an account, and your future trips to Kaweah Health will be stored there.

How to get a copy of a person’s birth or death certificate:

Birth and death certificates are not kept at Kaweah Health Medical Center, and Tulare County Clerk maintains them.
