The Johnson Health Center serves the communities of Lynchburg and surrounding Amherst, Bedford, and Campbell counties as a not-for-profit, autonomous Federally Qualified Health Center.

At least 51% of our patient population serves on the health center’s board of directors. Our goal is to establish lasting relationships with our neighbors that will allow us to provide medical treatment to anyone who needs it, regardless of their ability to pay.

A half-century ago, the first community health centers opened their doors to serve residents in neglected communities better.

Johnson Health Center is pleased to provide complete primary care to those in need of medical attention. All major insurance plans are accepted, and low or no-cost payment plans are available for those without insurance.

How to Make Information Requests

There are two ways to get a copy of your medical history.

Requesting your medical records may be done online using the My UNC Chart patient portal.

In-Person: Please bring a copy of your photo identification (driver’s license, passport, etc.) and the completed Authorization for Release of Information form.

The HIM Department is where you may get your hands on conditions and where they should be turned in. You may also submit a paper copy of the Authorization for Release of Information Form [PDF] in English or the Espaol Authorization Para Divulgar Informacion [PDF] in Spanish to the following address:

Our office will take 10–14 days to respond to your records request.


Sending your medical records straight to another doctor or hospital is free. Photocopying costs may be charged, however, if the copies are being made for the patient’s use.

Registry of Birth Certificates

Our birth registrar will submit the necessary paperwork for your child’s birth certificate with the local registrar within five days (as required by North Carolina Vital Records).

Birth Registration System

Please supply the baby’s full name and birth date if you or the other parent can. To ensure an accurate birth record, we also gather parental demographic data.

An unmarried father who wants his name included on his child’s birth certificate must submit a paternity affidavit.

After completing the birth certificate form, the mother will sign the original document. The next step is to deliver it to the state’s vital records office.

Making a Birth Certificate Copy

A certified copy of a birth certificate may be obtained from the state’s Department of Health and Human Services’ Office of State Vital Records.

You may get a copy of the birth certificate application by filling out the request section and sending it in as directed.

The Johnston County Register of Deeds is another source for certified copies of vital records, including birth certificates. The office may be found downtown Smithfield, within the Johnston County Courthouse at 207 E. Johnston St.

It’s Time to Go Home When

Johnson Health Center transportation

Amherst, Bedford, Campbell, Appomattox, and Lynchburg patients are eligible for free transportation to and from Johnson Health Center for medical visits.

The van is at your disposal from Monday through Thursday, 8:30 a.m. to 4:00 p.m. Specialty referral appointments are scheduled on Fridays.

When making a medical appointment, patients who need transportation should notify the phone service. This service is provided at no cost to you.


  • Amherst, Appomattox, Bedford, Lynchburg, and Campbell County residents are eligible for this service.
  • Transportation on Fridays to UVA or Carilion for specialty appointments
  • No cost to patients of the Johnson Health Center

Johnson Health Center Bedford

In January of 2014, Johnson Health Center inaugurated a new branch called Bedford Community Health Center.

Adults and children alike may get medical care, mental health treatments, a pharmacy, and obstetrics and gynecology at this facility.

The door is open for new patients at this clinic. Each and every insurance plan is welcome.

Patients without health insurance or inadequate coverage may qualify for a sliding scale price depending on their family size and annual income.
