How to Access Swope Health Portal and Get Your Medical Records

Since 1969, dependable medical treatment

Swope Health serves the Greater Kansas City area with general care and mental health services. NCQA has officially recognized us as a patient-centered medical home because of our dedication to using a team approach to medical treatment.

We have received approval from The Joint Commission and The Commission on Accreditation of Rehabilitation Facilities.

As President Lyndon Johnson’s Model Cities initiative required, we opened our doors in 1969 under the name Model Cities Health Corporation.

The original location of our clinic was in the crypt of the Metropolitan Missionary Baptist Church.

In our first year of operation, we serviced almost 2,000 patients on a budget of only $100,000 with a staff of 20. More than 40,000 people in western Missouri and eastern Kansas now get treatment from Swope Health.

We have been certified by the National Committee for Quality Assurance as a Federally Qualified Health Center and as a facility meeting the Federal Tort Claims Act requirements.

Our Aim

Swope Health is committed to enhancing community health and well-being by providing convenient, high-quality, all-encompassing patient care.

Swope Health Portal

Patient Portal for Swope Health Services

My name is Dan Gillen, and I’m a family nurse practitioner with Swope Health Services. I think our patient portal is excellent, and it’s a beautiful method of communication for both patients and doctors.

Swope Health Services’ patient portal functions similarly to email but in a secure environment that protects the privacy of your medical records.

If you send me a message via the portal, it will be stored in your medical record, where I will have access to it when I read and answer your message.

Send a message via the patient portal if you have a question that can’t wait until your next appointment.

I can promise you that it will reduce the frustrating game of “phone tag” that everyone plays when attempting to receive a prescription refill or a doctor’s recommendation.

It’s pretty simple to operate.

If you’re familiar with the Internet, the gateway will be a breeze. Please give one of our patient care employees your email address the next time you visit an SHS clinic.

We’ll send you an email with your login credentials (username and password) already entered.

Please find out more about our patient portal here.

Walk-In Dental Emergencies

Dental emergencies may be seen by appointment on a space-available basis at our Central, Independence, Northland, and Wyandotte locations during business hours.

The exact amount depends on dentists’ availability and the number of patients who have booked appointments.

There is always a queue outside the door by the time we open at 7:30 in the morning at Central. Let one of our uniformed guards know if you need immediate dental care.

It’s important to remember that our security personnel does morning walk-throughs, so the exact time you see them may vary. However, if you’re there for a dental emergency, make sure they know.

If you need emergency care at one of our other sites, please come in at the clinic’s opening hour.

If you do not obtain a number

If you don’t get an assigned number, you’ll be given a list of alternative dental safety net clinics from which to choose.

Swope Health does not recommend or guarantee the quality of dental care you get at any listed clinics or programs, and we’re only making a list available for reference purposes.

If you receive a number:

  1. You will be required to fill out a form for an appointment on the same day.
  2. Your form will be reviewed with you by a member of the dentistry department.
  3. If it’s found that you need immediate attention, you’ll be scheduled for an evaluation.
  4. The test for evaluation The evaluation of Swope Health will establish the severity of your ailment and the best course of therapy for you. The subsequent period of events varies from patient to patient:
  • In the case of an infection, antibiotic treatment must be finished before any more procedures may be performed. If the disease has been cleared up, we may go forward with your therapy.
  • If your dental care needs exceed those of Swope Health, you may be directed to an outside dental facility. In such cases, you are still responsible for paying the $45 assessment test price (sliding fee reductions may be available for those who qualify).
  • We promise to see you the same day if we can treat your problem. If your situation calls for further treatment, we will help you arrange for that before you depart.
