An overview of Erie Family Health Centers
Erie Family Health Centers started in 1957 when a kind doctor and an elderly patient both needed medical attention.
He is an internist at Northwestern University Medical School, and Carmela Jacob is his patient. Dr. Robert Snyder is the physician.
Residents of Chicago’s West Town neighborhood, including Carmela, were dissatisfied with the difficulties they experienced while attempting to get health care.
In response to an unmet need he saw in the area, Doctor Snyder started Erie Family Health Centers by offering on-site medical treatment at Erie Neighborhood House, a community center for immigrants in West Town.
The clinic started delivering senior health care services twice weekly with the help of Northwestern University medical students and community volunteers.
Students at Northwestern University’s Feinberg School of Medicine have voiced an increasing desire to staff a student-run clinic to offer low-income families access to medical care.
In 1968, Erie extended its programs to include low-income children and adults, making this ideal a reality.
At the time of incorporation in 1970, Erie had become a full-service community health facility and had separated from Erie Neighborhood House.
The Erie Senior Health Center was created in a high-rise residential complex for the elderly in the 1980s, bringing significant expansion and transformation.
In 1985, the Erie Teen Health Center was transformed from a squalid apartment clinic into a full-fledged health facility of its own.
When our first health facility relocated to Chicago Avenue in 1986, Erie Street was left behind. In 1986, Erie opened a full-service health clinic in Humboldt Park.
In honor of Erie’s services to the health and well-being of Chicago’s medically disadvantaged populations, Chicago’s mayor established October 6th Erie Family Health Center Day in 1988.
Increasing patient numbers need culturally competent, comprehensive health care as time goes on. More than 2,000 patients a month were being treated at our West Townsite by the end of 1987.
More than 82,000 patients a year are served by Erie’s 13 health clinics, which stretch from Chicago’s west side to Waukegan.
Four of the region’s most extensive primary care facilities provide integrated medical and dental health services, as do three additional extensive primary care facilities and the oldest and largest comprehensive teen and young adult health center in the region, and five health centers located in schools.
Dr. Snyder started providing medical treatment for West Town’s elderly over 60 years ago, and our objective hasn’t changed: to provide compassionate, culturally competent, and financially accessible health care to the Chicago region’s medically disadvantaged individuals.

A new patient portal for Erie
MyChart is the patient portal for Erie:
- The patient portal allows you to communicate with your doctor, obtain test results and referrals, download school physical forms, etc.
- In addition, the portal may be accessed using the MyChart app on your smartphone or tablet.
- As of March 22, 2022, all patients who had patient portal accounts before that date will also need to sign up for the new version.
- Inquire at the front desk or with your healthcare physician about registering for the portal.

They want to be ready for your arrival. As a result, making an appointment far in advance is essential. You will be asked for specific information when you schedule the appointment, so prepare it ahead of time:
- The patient’s first and last name
- The patient’s birth date
- The Erie location where you’d want service to be provided
- The name of the doctor or other provider you have previously visited (unless you can’t recall your provider’s name).
- A phone number where we may contact and/or text you to remind you of your appointment time.

Care for those in need in a community health setting! In addition, you’ll be in charge of overseeing the work of the Patient Care Services personnel, such as the Medical Assistants, who are part of a specialized team at the hospital.
