History

It was created in 1986 when the Sisters of Mercy Burlingame Regional Community and the Sisters of Mercy Auburn Regional Community combined their health care missions into one organization.

41,000 California Public Employees Retirement System members are covered by an Accountable Care Organization (ACO) founded by Dignity Health, Blue Shield, and Hill Physicians Medical Group in 2010. (CalPERS).

Until 2012, the corporation was a Roman Catholic Church-affiliated organization. When the corporate governance structure of the corporation was modified in 2012 to remove the Catholic Church from its ownership, the organization’s name was changed to Dignity Health.

The Dignity Health Arizona General Hospital in Laveen, Arizona, was created by a partnership between Adeptus Health and Dignity Health.

The Vatican blessed the merger of Dignity Health and CHI in 2018. On February 1, 2019, the union was finalized, under the new name of CommonSpirit Health, becoming the second-largest nonprofit hospital group in the United States.

The San Francisco Giants have chosen Dignity Health as their official healthcare provider.

Requests For Medical Record

Our Health Information Management (HIM) Department is responsible for keeping track of our patient’s medical data.

An individual patient’s medical history and progress are documented in a medical record that is accessible to all members of the healthcare team, including the patient’s physician.

Requests for a copy of a patient’s medical records must be submitted in writing using the forms provided below.

We ask that you please print out and complete the relevant form and bring it in person to the HIM department at the hospital where you had your treatment.

A picture ID, signature, or relevant questions will all be used by HIM to confirm your identity. We do this to ensure that your personal information is safe and secure. The business is open from 8 a.m. to 4:30 p.m., Monday through Friday.

Patients Should Know This Important Information

  • Patients may get a copy of their most current medical records by email. The ROI department may provide an email with a copy of recent records (within the previous two years).
  • The particular acknowledgment area must be checked and signed to obtain or authorize the following information. This includes AIDS/HIV, substance addiction therapy, and fertility treatment.
  • The Behavioral Health ROI permission form must be signed and returned with your request for a copy of your Behavioral Health record.
  • To obtain your medical records electronically, you must choose the option to receive them using Axway MailGate SC, Dignity Health’s secure email service. To access your medical data, you’ll be asked to establish a password and log in to your account. Please call our support center at (888) 307-0222 if you need assistance setting up a password.
  • For more than 20 paper pages, the Dignity Health Medical Foundation charges patients $15.00; for a flash drive, the fee is $10; and for a legally enforceable paper record, the price is $15.00. To avoid fraud, we only take checks as payment.
  • Recent records (within the previous two years) that are forwarded to you or your healthcare practitioner to ensure continuity of care are free.
  • Please describe the period for which you want the reports, as well as the specific words that you require.
  • On your ROI request, provide the names and locations of your service providers.
  • A patient’s medical records are given to a new doctor for the last two years unless otherwise specified.
  • The processing time for your request is 15 calendar days.
  • 10995 Gold Center Drive, Suite 290, 2nd Floor, Rancho Cordova, CA 95670, is where we’ll meet patients for pick-up. When you pick up your items, you’ll need to provide a valid picture ID.
  • This form may request a correction or amendment if you notice a mistake that needs to be addressed. Submitting a copy of each page in question and a completed form is essential. If your case is tough, you may have to wait an extra 30 days to obtain a written response.
