Hill Health Center was Connecticut’s first Federally Qualified Community Health Center, founded in 1968 by New Haven residents like Mr Cornell Scott in collaboration with Yale University’s School of Medicine.
Cornell Scott-Hill Health Center, as we are now called, has been serving the Greater New Haven area since it opened its doors decades ago.
Over time, we have increased the number of locations where we provide care and added additional offerings to meet better the needs of the individuals we support.
Cornell Scott-Hill Health Center is a national pioneer in community healthcare, providing cutting-edge, coordinated services at more than twenty locations throughout New Haven County.
We maintain Mr Scott’s fight for community healthcare and his commitment to solving the healthcare needs of the under-served in our minds as we treat over 36,000 people annually.

Definitions of Our Core Values
We opened in Connecticut in 1997 and are the state’s first community health centre.
We were founded in 1968 as a public-private partnership between the Yale School of Medicine and the surrounding neighbourhood.
Cornell Scott-Hill Health Center has served the New Haven areas for many years and is some of the most economically challenged in all of Connecticut.
In addition to serving the residents of West Haven, Ansonia, Seymour, Shelton, Naugatuck, and Oxford, Cornell Scott-Hill Health Center serves the surrounding communities of the Lower Naugatuck Valley.

Programs for Caring Communities that Serve Adults and Women
A new “one-stop-shop” primary care centre on Long Wharf has opened in the recently completed two-story, 52,000-plus square-foot medical building at 150 Sargent Dr Cornell Scott Hill Health Center is presently taking new patients and will be in charge of the adult and women’s care programs.
Mood disorders, anxiety disorders, drug addiction, eating disorders, trauma, developmental disorders, and disruptive behaviour disorders are just some of the many mental health issues that the Cornell Scott Hill Health Center treats on an inpatient basis for a short period.
- Prevention via Inoculation (for adults)
- Two-day interval, every other week, between Tuesday and Thursday
- During regular business hours, walk-ins and appointments are welcome.
- Appointments will be held until 4:45 p.m.
Medicine for Adults
Concern for your physical and mental wellbeing is a top priority for us. And we know that you place a high value on them as well.
Therefore, our physicians, APRNs, and PAs will collaborate with you to achieve your health and wellness objectives.
Whether you require a routine physical, treatment for an acute sickness, or ongoing management of a disease like diabetes, we can help.
We’ll work with you as an ally to ensure you receive the treatment you need. You may visit our convenient locations in New Haven County to speak with a local physician, and internal medicine and adult health are the focus of this educational resource.
