An overview of Community Health Choice

To better the health and well-being of Texans, Community Health Choice established a nonprofit managed care company to serve the community.

STAR Medicaid coverage for low-income children was first offered by Harris Health System, the region’s public, academic healthcare system, in 1997. On top of our extensive provider network, we provide a full range of reasonable and no-cost health insurance policies to choose from.

Nonprofit, Managed Care Organization (MCO) STAR and CHIP programs are offered through our local, nonprofit MCO.

With the help of the Health Insurance Marketplace, we also provide plans. It’s a nonprofit health plan based right here in our community.

More than 400,000 people have joined the community, a doctor and hospital network totaling more than 10,000.

Our goal is to make healthcare and health-related social services more accessible to Texans who are underserved. A community is a place where we care for and serve our neighbors.

Because we have earned our members’ and their families trust, we open doors to high-quality healthcare while also making the procedure as painless as possible for them and their loved ones.

In a year, a lot may happen. Your medical or personal circumstances may have altered. At the same time, newer versions of health insurance plans may provide you with more excellent coverage.

Open Enrollment in the Market

There is an open enrollment period for students from November 1, 2021, to January 15, 2022. Preparation pays off now.

  • For coverage to begin on January 1, 2022, you must enroll by December 15, 2021.
  • If you want coverage that begins on February 1, 2022, you must enroll between December 16, 2021, and January 15, 2022.

As your health care provider, we are available anytime you need us and assist you in discovering new methods to keep healthy. Letter and ID card ready for your arrival.

Make it official by following the instructions below. You may take advantage of free health checkups as soon as you’ve completed the registration process.

1. Pay the first installment.

You may pay for your premium in a variety of ways:

  • Quick Payments may be made by paying online.
  • You may pay by phone at 713.295.6704 or toll-free at 1.855.315.5386.
  • Visit one of the CheckFreePay Payment Centers listed below to make your payment in person.
  • Send a cheque or money order in the mail. To donate, please send your money to Community Health Choice, PO Box 844124, Dallas, TX 75284-4124.

Please be aware that bills are automatically mailed. Thank you in advance if you have already made a payment. The processing of certain payments may take up to a week.

2. Choose a primary care physician.

Community Health Choice’s network of primary care physicians may help you better organize your treatment. It’s easy to discover a new primary care physician through our Provider Directory.

Go to the “Message Center” and provide us the provider’s first and last name/clinic name and phone number. Alternatively, you may reach us by phone at 713.295.6704 or 1.855.315.5386 (toll-free).

3. Create a Member account on the internet.

Personalized care for you and your family. Managing your health insurance is a cinch with a My Member Account. Everything you could need is at your fingertips in this virtual world.

Five to seven business days after your initial membership payment has been received; new members may establish a My Member Account.
My Member Account allows you to do the following:

  • Set up auto-payments for insurance premiums and make payments online.
  • Your Member ID card may be printed out for safekeeping if you need it.
  • Check the status of your claim.
  • Our Member Services staff may be contacted through private messaging.
  • The list goes on and on and on.

4. Identify any supporting documents required.

To encourage people to sign up for health insurance via the Health Insurance Marketplace, the federal government offers the Advanced Premium Tax Credit (APTC), a subsidy of up to 400 percent of the FPL.

Subsidies assist in lowering the cost of health insurance by covering a portion of the premiums.
If the Health Insurance Marketplace has requested supporting paperwork, please provide it to the Health Insurance Marketplace’s office.

CMS (Centers for Medicare & Medicaid Services) will only accept financial assistance documentation with premiums if you provide it within 30 days after enrolling.

If you don’t deliver the papers that are still pending, you will no longer be eligible for Marketplace coverage.
